PATIENT INFORMATION

We want your visit to Orthopaedics PC to be as easy as possible.  Please print and complete the three forms listed below and bring them with you to your clinic appointment.  This will save you time at your visit.  If you have been a patient in the past, we ask you to print and complete these forms to update your medical chart. 

Please use full names, be specific as to how long you have been having trouble, indicate either right or left, etc.

Also, please bring to your appointment:

1. Insurance cards and necessary forms
If your insurance company requires pre-admission authorization for hospitalization and/or surgery, it is your responsibility to notify us and secure the necessary forms.

2. List of your medications

3. X-rays taken previously for this particular problem
Includes MRI, CT, bone scans, and regular x-rays.  Call the office or hospital where you had your x-rays taken and they will loan them to you or they will send them to us in advance.
